The Primarily Home™ program, provided by Baptist Health™, works together with your Baptist Health primary care provider (doctor, nurse practitioner, and/or physician assistant) to deliver the care you need to stay healthy. In collaboration with your primary care provider, the Primarily Home care team offers in-home clinical care as an extension of your existing primary healthcare team.

  • Visit with a Primarily Home care team member in your home who supports your primary care provider in overseeing your health and wellbeing.
  • Enjoy a personalized approach to your healthcare journey. Receive a complete review of your health history and current medications.
  • Move forward with a personalized care plan to help you understand your health choices and next steps.

WHO IS PART OF YOUR PRIMARILY HOME TEAM?

Your Primarily Home care team consists of an advanced practice provider (nurse practitioner or physician assistant), registered nurse, and patient engagement specialist who will provide you the support and guidance you deserve.

WHAT TO EXPECT?

After your in-home visit, the information gathered will be given to your primary care provider and the right care plan will be developed with you to fit your lifestyle and daily needs. The Primarily Home care team is available to answer questions and recommend solutions for any health-related concerns you might have. Your Baptist Health primary care provider will stay in close contact with your care team as we work together to keep you healthy.
